Log into your account: https://ultraluxhealth.com/my-account/
Once logged in, navigate to the side menu and click on “Orders”
Under your order, click the button that says “Leave a Review”
Select how many stars you’d like to leave for the product out of 5
Leave some comments about your experience with the product! What did you love about it? Have you had a good experience with our customer service? How has the product benefited your health journey? The more detailed, the better!
Once you’re satisfied, click “Submit”
Your $50 store credit (5,000 rewards points) will be added to your UltraLux Health account within 24 hours!
Bonus points: You can leave a review for every product you’ve purchased! So if you’ve bought multiple products from us, you can get even more rewards!
